OBJECTIVE: The paper describes the introduction, and early use of chemically and electrically induced convulsive therapies, at the Mysore Government Mental Hospital (MGMH), now the National Institute of Mental Health and Neuro Sciences, Bangalore, India. Cardiazol and ammonium chloride were used at MGMH before the introduction of electroconvulsive therapy (ECT). The study examines the early history, clinical correlates and outcome of convulsive therapies and attempts to contextualize how local conditions influenced implementation. METHOD: Three sets of archival case-records from 1938 to 1948, each of a period of 9 months following the implementation of a particular mode of convulsive therapy were reviewed. RESULTS: During the examined timeframe, 40 patients received cardiazol, 95 ammonium chloride and 50 unmodified ECT. Schizophrenia was the commonest clinical indication for convulsive therapy across all modalities of treatment. When outcomes were examined, 45%, 48.4% and 62% of patients were clinically reported to have been either cured/improved after receiving cardiazol, ammonium chloride and ECT respectively. Those receiving cardiazol had a high mortality of 22.5%, compared to 3.1% for ammonium chloride and 4% with ECT. CONCLUSIONS: Convulsive therapies were one of the first somatic psychiatric treatments, introduced around 1930s and 1940s all over the world, including in India. Our archival records suggest that many international ideas about somatic treatments were quickly adopted in India. Electroconvulsive therapy and other novel neuromodulatory interventions continue to be used and actively researched in India.

Objective-Associations between stressful life events (SLEs) during childhood and suboptimal healthcare access and use has been documented. Recent changes to the National Health Interview Survey's questionnaire enabled the inclusion of SLEs in the child sample, resulting in an additional national data source where SLEs can be tracked. In this report, the latest SLE estimates are examined for children aged 2-17 years in the United States and their associations with healthcare utilization. Methods-Data from the 2021 National Health Interview Survey were used to examine the percentage of children who experienced one or more SLEs-emotional abuse, unmet basic needs, experiences of racism, household mental illness, household substance abuse, parental incarceration, and exposure to neighborhood violence-and describe the association between SLEs and selected healthcare utilization indicators over the past 12 months (as in no well-child visit, emergency room visits, urgent care visits, unmet medical care needs due to cost, use of prescription medications for mental health, and use of any mental health therapy). Multivariate logistic regression models were fit to produce prevalence ratios for selected healthcare utilization indicators by SLEs, after adjusting for child and family sociodemographic characteristics. Results-In 2021, one in five children aged 2-17 years had ever experienced an SLE. In general, all SLEs were related to higher healthcare utilization (as in emergency department visits or mental health therapy) and unmet medical care needs. In general, no significant associations were found between experiencing SLEs and not receiving preventive health care. After adjusting for demographic characteristics, higher rates of healthcare utilization, unmet medical care needs, and mental healthcare utilization generally persisted for children with SLEs. Conclusion-This report expands knowledge on the relationship between childhood SLEs and the use of preventive care, healthcare utilization, and mental health care. National Health Interview Survey data can be used to monitor trends in these associations over time.

The effects of systemic insulin administration at different concentrations on the testicular tissue of diabetic adult rats, induced by streptozotocin, are evaluated by the morphological analysis of spermatogenic process. Twenty-four adult male rats were divided into 1) Control Group: they received citrate buffer, by intraperitoneal injection; 2) Diabetic Group: induced by intraperitoneal injection of streptozotocin (60 mg. kg-1 of body weight); 3) Insulin 50%: induced diabetes treated with half of standard dosage of insulin; 4) Insulin 100%: induced diabetes treated with standard dose of insulin. After eight weeks, animals were weighted and anesthetized; testicles were removed and processed in resin. Body and testicular weight of diabetic rats decreased when compared to that of control. Parameters increased with insulin therapy. Testosterone levels were low in diabetic animals but rates recovered after insulin therapy. Nuclear diameter and volume of Leydig cells decreased in diabetic rats although they significantly increased after insulin therapy. Results showed that the administration of insulin in diabetic rats promoted a protective effect of testicular parenchyma, enhancing efficient recovery on testosterone levels and increase in daily sperm production.

RESUMO

Despite decades of experience with ECT, no single agent has emerged as a suitable strategy to mitigate the associated neuro-cognitive deficits. In this first of its kind pilot randomised controlled trial, we sought to demonstrate the effect of yoga on neurocognitive deficits. We found a favourable effect of Yoga in the domain of verbal fluency, however our study did not reveal significant differences in other cognitive domains. Future studies could look to build on this study while addressing the limitations cited.

This paper examines the evidence behind the use and decline of insulin coma therapy as a treatment for schizophrenia and how this was viewed by the psychiatric profession. The paper demonstrates that, from the time of its introduction, there was considerable debate regarding the evidence for insulin treatment, and scepticism about its purported benefits. The randomized trials conducted in the 1950s were the result, rather than the origins, of this debate. Although insulin treatment was subsequently abandoned, it was still regarded as a historic moment in the modernization of psychiatry. Then, as now, evidence does not speak for itself, and insulin continued to be incorporated into the story of psychiatric progress even after it was shown to be ineffective.

O diabetes melito é o maior fator de risco para doença arterial coronariana. Além da longa duração de diabetes, outros fatores, como presença de doença arterial periférica e tabagismo são fortes preditores para anormalidades na cintilografia de perfusão do miocárdio. O objetivo deste estudo foi avaliar o impacto dos fatores de risco de pacientes diabéticos nos resultados da cintilografia de perfusão do miocárdio e comparar com os resultados de pacientes não diabéticos em uma clínica de medicina nuclear. Foi realizado um estudo transversal retrospectivo por meio da análise de prontuários de pacientes que realizaram cintilografia miocárdica no período de 2010 a 2019. Foram avaliados 34.736 prontuários. Analisando a fase de estresse da cintilografia de perfusão do miocárdio, os portadores de diabetes melito precisaram receber estímulo farmacológico duas vezes mais que os não diabéticos para sua realização. Também foram avaliados fatores que tivessem impacto negativo no resultado da cintilografia de perfusão do miocárdio, e foi visto que o diabetes melito (33,6%), a insulinoterapia (18,1%), a hipertensão arterial sistêmica (69,9%), a dislipidemia (53%), o sedentarismo (83,1%), o uso de estresse farmacológico (50,6%), a dor torácica típica (8,5%) e a angina limitante durante o teste (1,7%) estiveram associados significativamente (p<0,001) a anormalidades neste exame. Diabetes mellitus (DM) is the greatest risk factor for coronary artery disease. In addition to a long duration of diabetes, the presence of peripheral arterial disease and smoking are strong predictors of abnormalities on myocardial perfusion scintigraphy (MPS). This study aimed to assess the impact of risk factors in diabetic patients on MPS results and compare them with those of non-diabetic patients in a nuclear medicine clinic. A retrospective cross-sectional study was performed through the analysis of the medical records of patients who underwent MPS in 2010­2019. A total of 34,736 medical records were evaluated. Analyzing the stress phase of MPS, DM patients required two-fold more pharmacological stimulation than non-diabetic patients for MPS. Factors that negatively impact the MPS results were also evaluated, and DM (33.6%), insulin therapy (18.1%), systemic arterial hypertension (69.9%), dyslipidemia (53%), sedentary lifestyle (83.1%), use of pharmacological stress (50.6%), typical chest pain (8.5%), and limiting angina during the test (1.7%) were significantly associated (p < 0.001) with test abnormalities. RESUMO Objetivo: avaliar a utilização de diferentes estratégias de educação em saúde destinadas ao cuidado das pessoas com diabetes em tratamento de insulinoterapia. Métodos: revisão integrativa da literatura, com busca realizada no período de junho de 2021 e março de 2022, com recorte temporal de dez anos, nas bases eletrônicas Science Direct, Cumulative index to nursing and Allied Health Literature, portal PubMed, Web of Science, Scopus, Scientific Electronic Library Online e Biblioteca Virtual em Saúde, com descritores controlados indexados no Descritores em Ciências da Saúde e no Medical Subject Headings. Resultados: nos oito estudos selecionados a partir dos critérios de inclusão, as estratégias encontradas foram o uso de material impresso, a orientação verbal individual e em grupo, o emprego de recurso audiovisual e a simulação clínica. Conclusão: a análise dos estudos permitiu verificar que diferentes modalidades de educação em saúde são eficazes para auxiliar no gerenciamento do diabetes. Entretanto, os métodos de ensino passivos ainda são maioria, o que torna necessário que novos estudos explorem outras formas de preparo para o cuidado em saúde, como as metodologias ativas de ensino-aprendizagem.

Conclusión: el análisis de los estudios permitió verificar que diferentes modalidades de educación en salud son eficaces para ayudar en el manejo de la diabetes. Sin embargo, los métodos de enseñanza pasivos todavía son mayoría, lo que hace necesario que nuevos estudios exploren otras formas de preparación para el cuidado en salud, como las metodologías activas de enseñanza-aprendizaje.

Methods: integrative literature review, with a search conducted in June 2021 and March 2022, with a ten-year time frame, in the electronic bases Science Direct, Cumulative index to nursing and Allied Health Literature, PubMed portal, Web of Science, Scopus, Scientific Electronic Library Online and Virtual Health Library, with controlled descriptors indexed in Descriptors in Health Sciences and Medical Subject Headings. esults: in the eight studies selected based on the inclusion criteria, the strategies found were the use of printed material, individual and group verbal guidance, the use of audiovisual resources and clinical simulation. Conclusion: the analysis of the studies allowed us to verify that different modalities of health education are effective to assist in the management of diabetes. However, passive teaching methods are still majority, which makes it necessary for further studies to explore other forms of preparation for health care, such as active teaching-learning methodologies.

RESUMO

The first third of the twentieth century changed the therapeutical landscape with the emergence of new treatments for the mentally ill in asylums. However, the historiography of their use in Spanish psychiatric establishments has been scarcely studied. The popularization of barbiturate sleep therapies, insulin shock, cardiazol therapy, electroshock and leucotomy spread from the beginning of the century. However, the Spanish Civil War and Spain's isolation during Franco's autarky (1939-52) made their implementation difficult. Through historiographic research using medical records as documentary sources, this work analyses the socio-demographic conditions of the asylum population during the first decade of Franco's dictatorship. The treatments used in Leganés Mental Asylum are described and are compared with those used in other Spanish psychiatric institutions.

BACKGROUND: The aim was to study the effects of barbiturate coma treatment (BCT) on intracranial pressure (ICP) and intracranial compensatory reserve (RAP index) in children (< 17 years of age) with traumatic brain injury (TBI) and refractory intracranial hypertension (RICH). METHODS: High-resolution monitoring data were used to study the effects of BCT on ICP, mean arterial pressure (MAP), cerebral perfusion pressure (CPP), and RAP index. Four half hour long periods were studied: before bolus injection and at 5, 10, and 24 hours thereafter, respectively, and a fifth tapering period with S-thiopental between < 100 and < 30 µmol/L. S-thiopental concentrations and administered doses were registered. RESULTS: Seventeen children treated with BCT 2007-2017 with high-resolution data were included; median age 15 (range 6-17) and median Glasgow coma score 7 (range 3-8). Median time from trauma to start of BCT was 44.5 h (range 2.5-197.5) and from start to stop 99.0 h (range 21.0-329.0). Median ICP was 22 (IQR 20-25) in the half hour period before onset of BCT and 16 (IQR 11-20) in the half hour period 5 h later (p = 0.011). The corresponding figures for CPP were 65 (IQR 62-71) and 63 (57-71) (p > 0.05). The RAP index was in the half hour period before onset of BCT 0.6 (IQR 0.1-0.7), in the half hour period 5 h later 0.3 (IQR 0.1-0.7) (p = 0.331), and in the whole BCT period 0.3 (IQR 0.2-0.4) (p = 0.004). Eighty-two percent (14/17) had favorable outcome (good recovery = 8 patients and moderate disability = 6 patients). CONCLUSION: BCT significantly reduced ICP and RAP index with preserved CPP. BCT should be considered in case of RICH.

BACKGROUND: To compare the antidepressant effects and cognitive adverse effects of intravenous ketamine infusion and Electro-convulsive therapy (ECT) in persons with severe depressive episodes. METHODS: This assessor-blinded randomized control trial included 25 patients (either sex; 18-65 years) meeting ICD-10 criteria for severe depression (bipolar or unipolar). Patients received either ECT (n = 13) or intravenous infusions of ketamine hydrochloride (0.5 mg/kg over 45 min; n = 12) for six alternate day sessions over a period of two weeks. Severity of depression was assessed at baseline and on every alternate day of intervention using the Hamilton Depression Rating Scale (HDRS) and self-reported Beck Depression Inventory (BDI). RESULTS: Baseline socio-demographic and clinical variables including HDRS (ECT: 25.15±6.58; Ketamine: 23.33±4.05, p = 0.418) and BDI (ECT: 37.07±6.58; Ketamine: 33.33±9.29; p = 0.254) were comparable. Repeated-measures analysis of variance revealed that ECT patients showed significantly greater reduction in HDRS (group*time interaction effect; F = 4.79; p<0.001) and BDI scores (group*time interaction effect; F = 3.83; p<0.01). ECT patients had higher response rate than ketamine patients [HDRS: ECT- 13/13(100%) vs ketamine- 8/12 (66.70%); p = 0.04]. This was true for remission as well [ECT- 12/13(92.30%) vs ketamine- 6/12(50%), p = 0.030; both HDRS and BDI]. Performance on Digit Symbol Substitution Test (as part of the Battery for ECT-Related Cognitive Deficits scale) significantly improved in ketamine patients (p = 0.02) while that in ECT patients worsened non significantly (p = 0.30). LIMITATIONS: Relatively small sample size; higher proportion of dropouts in the Ketamine arm. CONCLUSION: This study favoured ECT over ketamine for a better efficacy over six treatment sessions in severe depression. The results need to be replicated in larger studies. In the treatment of depression, when pharmacotherapy, psychotherapy and the oldest brain stimulation techniques are deadlocked, the emergence of new therapies is a necessary development. The field of neuromodulation is very broad and controversial. This article provides an overview of current progress in the technological advances in neuromodulation and neurostimulation treatments for treatment-resistant depression: magnetic seizure therapy; focal electrically administered seizure therapy; low field magnetic stimulation; transcranial pulsed electromagnetic fields; transcranial direct current stimulation; epidural cortical stimulation; trigeminal nerve stimulation; transcutaneous vagus nerve stimulation; transcranial focussed ultrasound; near infra-red transcranial radiation; closed loop stimulation. The role of new interventions is expanding, probably with more efficacy. Nowadays, still under experimentation, neuromodulation will probably revolutionise the field of neuroscience. At present, major efforts are still necessary before that these therapies are likely to become widespread.Key pointsThere is a critical need for new therapies for treatment resistant depression.Newer therapies are expanding. In the future, these therapies, as an evidence-based adjunctive treatments, could offer a good therapeutic choice for the patients with a TRD.The current trend in the new neuromodulation therapies is to apply a personalised treatment.These news therapies can be complementary.That treatment approaches can provide clinically significant benefits.

Background: Treatment-resistant bipolar depression can be treated effectively using electroconvulsive therapy, but its use is limited because of stigma and cognitive adverse effects. Magnetic seizure therapy is a new convulsive therapy with promising early evidence of antidepressant effects and minimal cognitive adverse effects. However, there are no clinical trials of the efficacy and safety of magnetic seizure therapy for treatment-resistant bipolar depression. Methods: Participants with treatment-resistant bipolar depression were treated with magnetic seizure therapy for up to 24 sessions or until remission. Magnetic seizure therapy was applied over the prefrontal cortex at high (100 Hz; n = 8), medium (50 or 60 Hz; n = 9) or low (25 Hz; n = 3) frequency, or over the vertex at high frequency (n = 6). The primary outcome measure was the 24-item Hamilton Rating Scale for Depression. Participants completed a comprehensive battery of neurocognitive tests. Results: Twenty-six participants completed a minimally adequate trial of magnetic seizure therapy (i.e., ≥ 8 sessions), and 20 completed full treatment per protocol. Participants showed a significant reduction in scores on the Hamilton Rating Scale for Depression. Adequate trial completers had a remission rate of 23.1% and a response rate of 38.5%. Per-protocol completers had a remission rate of 30% and a response rate of 50%. Almost all cognitive measures remained stable, except for significantly worsened recall consistency on the autobiographical memory inventory. Limitations: The open-label study design and modest sample size did not allow for comparisons between stimulation parameters. Conclusion: In treatment-resistant bipolar depression, magnetic seizure therapy produced significant improvements in depression symptoms with minimal effects on cognitive performance. These promising results warrant further investigation with larger randomized clinical trials comparing magnetic seizure therapy to electroconvulsive therapy. Device-based neuromodulation of brain circuits is emerging as a promising new approach in the study and treatment of psychiatric disorders. This work presents recent advances in the development of tools for identifying neurocircuits as therapeutic targets and in tools for modulating neurocircuits. We review clinical evidence for the therapeutic efficacy of circuit modulation with a range of brain stimulation approaches, including subthreshold, subconvulsive, convulsive, and neurosurgical techniques. We further discuss strategies for enhancing the precision and efficacy of neuromodulatory techniques. Finally, we survey cutting-edge research in therapeutic circuit modulation using novel paradigms and next-generation devices.

RESUMO

RESUMO Objetivo: Analisar a taxa de reuso de seringas e agulhas e identificar as práticas de descarte de perfurocortantes por usuários de insulina. Método: Estudo transversal, realizado com pessoas diagnosticadas com diabetes mellitus tipo II de um Centro de Referência em Diagnóstico e Terapêutica da região Centro-Oeste do Brasil. Foram utilizados dados secundários provenientes de prontuários e fichas de consulta. As diferenças de proporção foram estimadas pelo teste qui-quadrado, considerando-se p<0,05. Resultados: A taxa de reuso de seringas e agulhas foi de 94,9%. Houve alta prevalência do reuso de seringas e agulhas por mulheres, com mais de 60 anos, com tempo de estudo menor do que cinco anos, que possuíam mais de 11 anos de diagnóstico. A maioria dos usuários descartou as agulhas e seringas no lixo domiciliar. Tais dados evidenciam que as práticas de autocuidado realizadas pelas pessoas com DM quanto ao reuso e descarte de perfurocortantes não são seguras e podem favorecer complicações. Conclusão: Apesar das características demográficas e clínicas não terem apresentado associação com o reuso, esforços devem ser direcionados para as práticas de educação em saúde, levando em consideração o contexto da pessoa, a condição socioeconômica, nível de escolaridade e seu papel ativo no seguimento do tratamento.


RESUMEN Objetivo: analizar la taza de reutilización de jeringas y agujas e identificar las prácticas de desecho de punzocortantes por usuarios de insulina. Método: estudio transversal, realizado con personas diagnosticadas con diabetes mellitus tipo II de un Centro de Referencia en Diagnóstico y Terapéutica de la región Centro-Oeste de Brasil. Fueron utilizados datos secundarios provenientes de registros médicos y fichas de consulta. Las diferencias de proporción fueron analizadas por la prueba de chi-cuadrado, considerándose p<0,05. Resultados: la taza de reutilización de jeringas y agujas fue de 94,9%. Hubo alta prevalencia de la reutilización de jeringas y agujas por mujeres, con más de 60 años, con tiempo de estudio menor que cinco años, que poseían más de 11 años de diagnóstico. La mayoría de los usuarios descartólas agujas y jeringas en la basura domiciliaria. Tales datos evidencian que las prácticas de autocuidado realizadas por las personas con DM respecto a la reutilización y desecho de punzocortantes no son seguras y pueden favorecer complicaciones. Conclusión: Aunque las características demográficas y clínicas no hayan presentado asociación con la reutilización, esfuerzos deben ser dirigidos para las prácticas de educación en salud, teniendo en cuenta el contexto de la persona, la condición socioeconómica, el nivel de escolaridad y su rol activo en el sector del tratamiento.


ABSTRACT Objective: To analyze the rate of reuse of syringes and needles and identify the practices of disposal of sharps by insulin users. Method: Cross-sectional study conducted with people diagnosed with type II diabetes mellitus at a Reference Center for Diagnosis and Therapy in the Midwest region of Brazil. Secondary data from medical records and consultation forms were used. Differences in proportions were estimated by the chi-square test, considering p < 0.05. Results: The rate of reuse of syringes and needles was 94.9%. There was a high prevalence of reuse of syringes and needles by women, people aged over 60 years, with less than five years of schooling, and more than 11 years of diagnosis. Most users discarded needles and syringes in domestic waste. Thus, self-care practices of people with DM regarding the reuse and disposal of sharps are not safe and may favor complications. Conclusion: Althoughdemographic and clinical characteristics had no association with reuse, efforts should be directed towards health education practices, taking into account the person's context, socioeconomic condition, educational level, and active role in the follow-up treatment.

Contexto: Nas décadas de 1920-40, a psiquiatria tradicional no Brasil apostava na prática de convulsoterapias como tratamento inovador para reduzir sintomas das doenças mentais. Objetivo: Analisar as técnicas de aplicação de convulsoterapias em dois periódicos médicos. Metodologia: Estudo documental, qualitativo, cujas fontes históricas foram os Arquivos Brasileiros de Neurologia e Psiquiatria (ABNP) e os Anais do Instituto de Psiquiatria (IPUB) de 1928 a 1947. Para análise crítica foi feita a triangulação dos dados e organização temática. Resultados: Foram encontradas 27 publicações em ambos os periódicos. As publicações encontradas foram agrupadas de acordo com as técnicas utilizadas [choque cardiazólico (14); malarioterapia (5); choque insulínico (4); choque cardiazólico associado ao choque insulínico (2); eletroconvulsoterapia (1); choque cardiazólico associado a eletroconvulsoterapia (1)]. Conclusão: Os periódicos pesquisados contribuíam para a difusão do desenvolvimento científico sobre as terapias biológicas, publicando estudos que norteavam a sua aplicação, visando estabelecer um sentido entre as doenças mentais, os seus sintomas e os efeitos de cada uma das técnicas convulsivas de tratamento, sem mencionar explicitamente a participação da enfermagem.


Context: From 1920 to 40, traditional psychiatry in Brazil focuses on convulsive practices as an innovative treatment to reduce symptoms of mental illness. Objective: To analyze the techniques of application of convulsive therapies disclosed in two major medical journals. Methodology: Documental study, with a qualitative approach. The historical sources were the Brazilian Archives of Neurology and Psychiatry (ABNP) and the Annals of the Institute of Psychiatry (IPUB) from 1928 to 1947. Critical analysis was based on data triangulation and thematic organization. Results: Twenty-seven publications were found in both journals. The publications were gathered in groups according to the techniques used [cardiazolic shock (14); malariotherapy (5); insulin shock (4); cardiazolic shock associated with insulin shock (2); electroconvulsive therapy (1); cardiazolic shock associated with electroconvulsive therapy (1)]. Conclusion: The journals had contributed to the dissemination of scientific development on the biological therapies, with the publishing of studies to guide their implementation. They aimed to establish correlations between mental illnesses, their symptoms, and the effects of each convulsive treatment technique, without any explicit reference to the participation of nursing professionals.

RESUMO

Apresentar as manifestações de crianças com Diabetes mellitus tipo 1 participantes em uma sessão de Brinquedo Terapêutico Instrucional-BTI sobre a insulinoterapia. Estudo de Caso qualitativo, com análise temática indutiva, realizado em um acampamento de diabetes em 2014, com participação de duas crianças em idade escolar em sessão de BTI. As manifestações permitiram compreender que o domínio da insulinoterapia é um assunto complexo para a faixa etária, e demanda reforço contínuo visando a adaptação cognitiva da criança. A sessão de BTI oportunizou identificar o conhecimento prévio e as necessidades de aprendizagem sobre a conservação e técnica de injeção de insulina das crianças, com uso de linguagem adequada como o brincar. O BTI foi instrumento facilitador para identificar necessidades educacionais, além de sua ação assistencial. Seu uso reforça o potencial do lúdico na promoção do autocuidado relacionado à insulinoterapia da criança com diabete


To describe the expressions presented by children with type 1 diabetes who participated in a session on insulin therapy using the instructional therapeutic play (ITP). This is a qualitative case study in which inductive thematic analysis was conducted at a diabetes camp in 2014, and two school-aged children participated in an ITP session. The results revealed that children of the studied age group have difficulty mastering insulin therapy and need continued efforts to adapt cognitively to insulin therapy. The ITP session provided insight into the children's prior knowledge and their learning needs regarding the insulin storage and insulin injection technique using age-appropriate language such as playing. The ITP helped to identify educational needs and the involved care. The use of ITP strengthens the potential of play-based strategies to promote self-care related to insulin therapy in children with diabetes.
